In the United States, an estimated 16 million persons had COPD in 1994, a 60%

increase from 1982. In 1993, COPD was the fourth most common cause of death,

causing 95,910 deaths, more than twice the 47,335 deaths in 1979. For unknown

reasons, only about 15% of cigarette smokers develop clinically significant COPD. In

COPD patients with FEV1, less than 0.75 liters (about 20% of predicted), the mortality

rate is 30% at one year and 95% at ten years.

A study from Northern California showed a strong association between smoking and

adult respiratory distress syndrome (ARDS), a rapidly progressive syndrome of lung

injury with an overall fatality rate of 40 to 60%. The authors concluded that

approximately 50% of ARDS cases were attributable to cigarette smoking.
